Our goal is to facilitate a link between the healthcare team, insurance company and community based agencies by providing a plan of care that best meets the needs of each individual patient. We are available to answer any questions and provide information on discharge plans or other concerns during each patient’s hospital stay. Our highly trained staff personnel include Registered Nurses, Social Workers and clerical support. The Case Management and Social Services team demonstrates the power to educate, collaborate, advocate and communicate daily in solving the healthcare puzzle. |
