Once you have successfully completed your initial 90-days at CRMC, all full-time employees are eligible for Comprehensive Medical Insurance administered by Meritain Health. Visit www.mymeritain.com to setup an online account with Meritain so you can view claims and EOBs and take advantage of other services provided.
Visit http://www.georgiahealthnetwork.com/ to find a doctor in Network. When asked to provide a Plan name, please use: Open Choice PPO. Also keep in mind that our In-Network facilities are CRMC and St. Joseph/Candler in Savannah. South Georgia Medical Center in Valdosta can be used for cardiology, neurology and oncology services.
Healthy Merits Program
Healthy Merits is a wellness program provided in conjunction with CRMC’s medical plan through Meritain Health. Healthy Merits provides plan members with incentives in the form of cash rewards that can be used only for medical expenses. Flexible Spending Account
The Flexible Spending Account is an option to place tax-deferred money into an account that can be used only for medical purposes. An FSA is a “use-it-or-lose-it” account. Download the form, or call 1.800.748.0003.
Health Savings Account
If you are enrolled in CRMC’s High Deductible Health Plan, you have the option to contribute to a Health Savings Account. The HSA is an interest-bearing savings account that rolls-over from year to year. Money in this account can only be used for medical expenses. Prescription Drug Coverage
Your prescription drug coverage with Meritain is administered by Express Scripts. As a medical plan participant you have the option to purchase your prescriptions at CRMC’s in-house pharmacy as well.
You may enroll in Dental insurance as a stand-alone policy or in conjunction with your medical insurance. Individual and family coverage is available. The Plan is administered in conjunction with your medical benefits by Meritain. For more info visit www.mymeritain.com or call 1.888.300.3374.
Vision insurance at CRMC is a stand-alone, voluntary policy offered through VSP. Visit www.vsp.com for information about your benefit and look for an in-network VSP doctor.
Dependent Care Savings Account
The DCSA is an option to place tax-deferred money into an account that can be used only for your child’s daycare expenses. Submit your daycare receipts to Meritain and you will be reimbursed from your account of tax-deferred money.
Employer Paid Term-Life Insurance
Full-time employees are provided a $30,000 term-life insurance policy through UNUM. Spouses covered on the Medical Plan are provided a $5,000 policy and dependent children are provided a $2,000 policy. All policies are paid for entirely by CRMC.
401(K) Savings Retirement Plan
Full-time, part-time and temporary employees are eligible for CRMC’s 401(k) administered by Fidelity Investments. Plan highlights include:
